Simply call us at (508) 393-4544 or email Barbara Ann Dwyer, our Patient Liasion, at babs@drpoulos.com. We'll ask you for

some basic information and schedule a consultative appointment for you to meet with Dr. Poulos. Dr. Poulos tends to book several weeks to months in advance, so please plan accordingly!

If there is any likelihood that insurance may cover all or a portion of your consultation or surgery, we ask that you obtain a referral from your Primary Care Physician well in advance and fax it to our office at (508) 393-4692.

A consultation with Dr. Poulos is $100. Should you choose Dr. Poulos as your cosmetic surgeon, the $100 is deducted from the total cost of surgery and is automatically factored into the Proposal.

If insurance will cover the cost of your consultation, we will ask you for your co-pay at the time of your visit.

Please note, per our Cancellation Policy, all new cosmetic surgery patients must provide credit card information to schedule your initial appointment. Your credit card information is kept secure at all times. The card is kept on file and charged only if you cancel your appointment with less than 24 hours notice, arrive very late to your appointment, or miss your appointment altogether without adequate notice. In that case, the credit card will be billed a $100 "No-Show" fee.

Save Time and Shorten the Wait - Classique Patient Forms

At check-in, you will be asked to complete numerous documents detailing your medical history.  We've attached some of these forms here, so that you may save time and shorten the wait in the waiting room on the day of your appointment.  Please print them out and complete the attached forms at home, then bring them with you to your appointment. 

On the Day of Your Appointment

Once you arrive to your consultative appointment, you'll complete paperwork detailing your demographics, medical history, and goals to improve your personal appearance. Even though cosmetic surgery is oftentimes not covered by insurance, we will ask to photocopy your insurance card to keep on file. You'll watch a video on the procedure in which you've expressed interest. Then, our Nurse will lead you into an exam room where you'll change into a johnnie to meet with Dr. Poulos. Once you and Dr. Poulos arrive at a definitive surgical plan, Barbara Ann will present you with a Proposal outlining the surgeon's, anesthesiologist's, and hospital's fees. In the case of office procedures, the proposal will include surgeons's and facility fees.

A 10% deposit is required to book any cosmetic surgery. The remaining balance is paid two weeks' prior to surgery. Dr. Poulos accepts cash, bank check, VISA, MasterCard or AmericanExpress credit cards, or financing through Chase Health Advance or Care Credit. Please check with the hospital to learn when to pay your hospital and anesthesia fees, and what forms of payment are accepted for those fees.

Reconstructive or insurance-based surgeries typically take 6-8 weeks to receive either approval or denial by your insurance carrier. We wil inform you of your claim status as soon as we receive word from your insurer.
